Co-signing
When co-signing a note, a different attestation is needed as the current general attestation starts by saying the note is "truthful, accurate" which is something only the person who conducted the session can attest to. Supervisors are required to sign notes as part of supervision but cannot, legally, be required to say the note is truthful and accurate as they did not conduct the session. This language has caused issues as supervisors do not want to sign notes and be held liable if the note later is identified as fraudulent. Therefore, I request that a different attestation be available when signing as a supervisor on progress notes.
